Hope Hospice provides compassionate hospice care and support for individuals and families facing serious, life-limiting illness. Our services include personalized hospice care, caregiver support, grief and bereavement services, and community education and resources.
Both palliative and hospice care focus on comfort and quality of life. Palliative care can be provided at any stage of a serious illness and alongside treatments intended to cure or manage the condition. Hospice care is designed for people with a life-limiting illness when the focus of care shifts primarily to comfort, dignity, and quality of life rather than curative treatment.
A physician or other qualified healthcare provider is involved in determining eligibility for hospice care, but you don’t need a referral to start a conversation with Hope Hospice. Patients, family members, and caregivers can contact us directly with questions. Our team can explain hospice eligibility, discuss your situation, and help you understand the next steps.
Absolutely. Patients and families are important members of the care team. We encourage family members and caregivers to ask questions, participate in care planning, and communicate openly with our team. Care is shaped around each patient’s individual goals, values, needs, and preferences.
Hospice care supports the entire family, not just the patient.
