Advance Healthcare Directives

An advance healthcare directive, also known as living will, is a set of written instructions that a person gives that specify what actions should be taken for their health, if they are no longer able to make decisions because of illness or incapacity.

Physician Orders for Life-Sustaining Treatment (POLST)

POLST forms are available from Hope Hospice directly, pre-printed on the designated Pulsar Pink card stock. Call (925) 829-8770 to have one mailed to you, or you may pick the form up from our office in Dublin, CA.

So that healthcare providers can easily see a POLST form, it is suggested that the form be printed on 65# cover Pulsar Pink card stock. The POLST form will be honored on any color paper. Faxed copies and photocopies are also valid POLST forms.
