At Hope Hospice we are dedicated to supporting individuals with compassion, dignity and excellence.

Hospice is a special kind of care. It’s a gift you give yourself and those you love to ease your mind and provide relief. Hope Hospice offers our patients and their families the compassionate care, insight and guidance to help them navigate their way through the end of life with dignity.

As a proven leader in hospice care since 1980, our primary focus is on the well-being of the patient and their family. We meet patients “where they are” by respecting their values and decisions. In caring for patients, we develop a customized, family-centered plan of care which is continually updated. We pay attention to details that matter and anticipate our patients’ needs. It’s a collaboration that ensures patients and families get the care, respect and compassion they deserve.

Our committed and compassionate team of healthcare professionals, many with hospice and palliative care certification, provide a circle of support that addresses all of an individual’s needs: physical, mental, emotional and spiritual. Dedicated to making a meaningful impact and supported by the breadth of the experience, our staff offers the right amount of care when it’s needed most.

Hope Hospice delivers care wherever the patient resides whether it be at home, in an assisted living facility, a nursing home or other residential care facility in Alamo, Blackhawk, Castro Valley, Diablo, Danville, Dublin, Livermore, Pleasanton, San Ramon, and Sunol and the surrounding communities in the East Bay of San Francisco.

Our care team members are available around the clock – 24/7, and respond quickly to the patient’s and the family’s needs, requests and inquiries.
