Nurse Case Manager

Hope Hospice, Inc. is a non profit, non sectarian organization based in Dublin, California that provides compassionate, quality end-of-life care and grief support to patients and families in its San Ramon and Tri-Valley service areas.

We are seeking a full-time Nurse Case Manager to join our patient care team. This position will be responsible for identifying patient and family needs and for providing skilled nursing assessment, planning, and supportive care, and other appropriate services in accordance with the attending physician’s orders and the interdisciplinary group plan of care.

Qualifications required:

  • Licensed as Registered Nurse in the state of California
  • Minimum two years of recent acute care experience
  • Home Health/Hospice experience preferred
  • Strong clinical assessment, organizational, and communication skills
  • Basic computer skills

Admissions Nurses

Hope Hospice, Inc. is a non profit, non sectarian organization based in Dublin, California that provides compassionate, quality end-of-life care and grief support to patients and families in its San Ramon and Tri-Valley service areas.

We are looking for both full-time and part-time Admissions Nurses to support our patient care team in our admissions office. These positions will be responsible for the admission of patients to Hope’s program of care. The Admissions Nurse assists with the entire admissions process through assessment, planning, and coordination of patient and family needs in consultation with the attending physician and other hospice staff members.

  • Licensed as Registered Nurse in the state of California

  • Home Health/Hospice experience preferred

Dietitian - Per Diem 2.5 hrs/wk commitment

Reponsibilities:

  • Assesses and evaluates the nutritional status of the hospice patient and makes appropriate recommendations for diet modification. Provides In-service education to hospice staff and educational tools for patients/families.

Reports to:

  • Director of Patient Services

Functions:

  • Assists the physician, nurse and other hospice personnel in evaluating the dietary needs of the hospice patient.
  • Assists the patient/family to understand, accept, and follow dietary modifications as ordered by the physician.
  • Observes, records and reports to the physician, Director of Patient Services and the Case Managing hospice RN; the patient's reaction to dietary treatment and any related changes in the patient's condition.
  • Instructs, supervises or counsels other members of the hospice group including, when appropriate, Home Health Aides and family members, regarding the dietary care of the patient.
  • Attends Interdisciplinary Team meetings when appropriate and participates in the development and modification to the plan of care.
  • Attends in-services and education workshops when appropriate and takes additional training to promote professional growth.
  • Performs other assignments as directed.

Qualifications:

  • Possess a registration as a Dietitian and meets the eligibility requirements from The American Dietetic Association.
  • Minimum of one year's experience as a dietitian preferably in a hospice/home care setting.
  • Ability to assess and respond to the physical and emotional needs of the terminally ill patient and family in the home setting, and can demonstrate an understanding and tolerance for the patient's way of life.
  • Possess a valid California driver's license, valid auto insurance, and an automobile for field use.
  • Possess current CPR Certification.
  • Participates in the agency's Performance Improvement program as appropriate.

 

After Hours Registered Nurse

Full Time & Part Time After-Hours Nursing positions

Reports to the Director of Patient Services

 

Join our excellent team as an After Hours Nurse!

 

Responsibilities:

  • Be on-call from your home on scheduled nights from 5:00 pm to 9:00 am.  Enjoy a 3 day weekend off, every other weekend..

  • Make visits to patients in the home setting in accordance with the Interdisciplinary Plan of Care.

  • Be paid a guaranteed nightly pay, plus hourly pay for visits made over the nightly minimum.

Requirements:

  • Possess a current license to practice as a Registered Nurse in California.

  • Minimum of two years of varied work experience as a professional nurse, with preferably one of these years in an acute, oncology, Home Health and/or Hospice setting.

  • Strong clinical assessment, excellent communication and critical thinking skills.

  • Basic computer skills.
